While the orientation of the device probably isn't that important, you will probably get best results by having the dome parallel with the ceiling.

I agree, the dome never comes off mine. It helps to throw some of the
diffused light forward. I've used mine quite extensively now at weddings and other shoots and came to the same conclusion as Orangepeel after a lot of trial and error.
Have a look at the Gary Fong website for some good advice on how to use it.
