Background Help Ps Cs3

I need to know what I can do to get rid of all the clutter in the background. This is the last known photograph of my Grandma while she was alive and I want to redo this photograph. Where do i start and what can i do to get this photo completely clutter free ?

one option is to use the Quick Mask in Photoshop, heres a quick attempt, but once she's masked you can use any background you want. I wasn't to careful on the edges since I just wanted to show what you can do.

I would use the quick mask option as well but would use a brighter coloured background than that blue as she blends in to much.
