Can anyone tell me or show me how to use layers to change the background on this?
The picture wasn't a planned one hence the background being totally out of place!
I also struggled to cropped it to make the person the main feature.

Um? I think you will find the "cut out and replace" the background is not easy. You may wish to darken bits. You may wish to blur the background to enhance the depth of field effect. I would also consider darkening the elbow, if its in the final crop. Also conside vinyetting so corners are darker. The more you succeed in making the center the subject the greater the vinyetting you can use.

One option is to convert to Mono to lose distraction of colour in background. Cropped & slight S Curve added to boost contrast.

snap2photo - what's vinyetting? I'm pretty sure it doesn't involve wine..... (sorry bad joke)

I have no idea if I spelt it correct. Its dark bits arround the edge. Can help guide the view to the subject. Can give an old photo effect. Useful for removing unwanted, distracting background.

I think this one has a bit of everything.
Levels adjustment layer.
Vinyette. (HSL adjustment layer with layer mask)
Blur background. (Using groups in elements)
Remove elbow and cheak highlight.
Sharpen with a bit of threshold.
